2. The needle is going on at an angle

3. The paper tab or outer cap isn't positioned correctly

7. The needle or pen tip is damaged

A pen that's been dropped can have a cracked tip or bent threads. A pen needle from a crushed box can also have a deformed hub. Try a needle from a different, undamaged package. If several fresh needles still won't attach, the problem is likely the pen. Contact your pharmacy or the pen maker for a replacement instead of taping or forcing a needle on.
